We're joined by Alex Fleming, Dementia Operations Manager for the Sandwell Community Dementia Service.

Nominated for "Best Dementia Care Team" at the upcoming NDC awards, and recently commissioned for another 3 years' service, they provide dementia advisors and more to over 1,500 people.

As well as Bel Connolly from Leeds & York NHS FT, who leads the Memory Support Worker team, in partnership with Alzheimer's Society, to deliver more joined-up and person-centric care
